Position Overview : As a Care Coordinator at Kare Plus, you will play a crucial role in facilitating and coordinating support services for young people and families facing various challenges. You will work closely with clients, social workers, and other members of staff to ensure a personalised approach to care.

Responsibilities :

  1. Client Assessment: Conduct thorough assessments to understand the unique needs and challenges of clients.

  2. Coordination of Services: Collaborate with our social workers, healthcare assistants, and support workers to develop and implement comprehensive care plans.

  3. Advocacy: Advocate for clients to ensure they receive the necessary support and resources to improve their quality of life.

  4. Communication: Assist the Care Manager with maintaining open and effective communication with clients, their families, and external agencies involved in the care network.

  5. Crisis Intervention: Respond to crises and emergencies by coordinating appropriate interventions and support services. To be on call in case of emergency two weeks per month.

  6. Scheduling: Assist the Scheduler with the upkeep of the weekly timetable and shift allocations.

  7. Risk assessments: Conduct risk assessments as part of the individual's needs and update where required.

  8. Audits: Conduct visits to service users completing audits to ensure standards of care are always met.

  9. Liaise closely with Care Coordinators regarding recruitment, permanent allocation of Care & Support Workers, and the writing of Care & Support Agreements.

  10. Administration: To ensure that all written reports and administration is up to date. General administration and recruitment where necessary.

  11. To assist with employee appraisals and team meetings.

Requirements :

  • Access to own transport and full drivers license.
  • Ability to work on call for two weeks per month.
  • Bachelor's degree in social work or a related field required.
  • At least 1 year experience in similar role (preferred).
  • Excellent communication, interpersonal, and organisational skills.
  • Good IT skills, word, excel and PowerPoint.
  • Ability to work collaboratively in a team and independently as needed.
  • Full understanding of Health and Social Care.
