COMMUNITY CARE CO ORDINATOR - PERMANENT FULL TIME CONTRACT LOCATIONS - GALWAY CITY AND TUAM The role of the Community Care Coordinator is a pivotal role in supporting individuals to actively live in their community, ensuring their day-to-day delivery of services is meeting the changing needs of the people in the service. The Community Care Coordinator will work as part of a multi-disciplinary team in conjunction with the Area Manager. The Community Care Coordinator must have the capacity to adapt, modify and accordingly adjust to the changing environment for Disability services. They must plan to adapt and deliver a more holistic model of service that includes both health and social care. This is an exciting opportunity for a suitably qualified nurse to contribute to a new community based integrated multidisciplinary team supporting children and adults with needs arising from a combination of intellectual disability, health issues, mental health difficulties and/or behavioural challenges and other neuro-developmental disabilities. Applications are invited for this position from suitably qualified candidates. This role requires a strong ability to work with a team and to problem solve and work on your own initiative. The successful candidate must provide leadership in the development of the service in line with Person Centred Planning, HIQA standards, new directions, and future service requirements. Essential Criteria: Be a registered nurse on the live Register of Nurses kept by Nursing and Midwifery Board of Ireland (NMBI). Be registered in the Intellectual Disability Division of the Register of Nurses kept by NMBI/be registered in the Mental Health Division of the Register of Nurses kept RNID/RGN/RMHN qualification is desirable. OR   In exceptional circumstances, which will be assessed on a case-by-case basis be registered in another Division of the Register of Nurses and Midwives. An appropriate management qualification is desirable. A Full clean manual Driving Licence, which qualifies you to drive manual transmission vehicles The successful candidate must have experience of working with people with an intellectual disability. Understanding of an integrated day programme in a residential setting is an advantage. Understanding of community networking, supporting individuals to partake in activities in their local community, health care planning, goal setting and programme implementation, is desirable. Working with people with complex medical needs, physical disabilities, dementia, behaviours that challenge, and mental health is an advantage. Experience in leading, managing, and supporting staff teams is essential. Candidates must have the ability to exercise good judgement, discretion, and confidentiality, to communicate effectively in both written and verbal forms and to co-operate in a team environment with a person-centred philosophy. Report writing and record keeping is an essential part of the post.
