Caring duties specific to the client will be identified by the Public Health Nurse, Community Registered General Nurse (PHN/CRGN) Team. Client specific duties will be defined by the Home Support Resource Manager in consultation with PHN/CRGN Team and client. The HSCA will work as part of the local team, will be rostered to work on a 5/7 basis Monday through to Sunday as required, may be required to work split-shifts, will work assignments as allocated by line manager.</p> <p></p> <p><strong>Health Care Assistant, Home Support - Job Specification, Terms &amp; Conditions</strong></p> <p></p> <p><strong>Principal Duties and Responsibilities</strong></p> <p></p> <p><strong><em>To work under the direction of the Home Support Resource Manager to provide personal care services specific to the client’s needs such as:</em></strong></p> <ul> <li>Bathing</li> <li>Showering</li> <li>Dressing</li> <li>Mouth care</li> <li>Shaving</li> <li>Hair care</li> <li>Toileting (emptying and cleaning of commode, urinal)</li> <li>Continence care</li> <li>Mobilising</li> <li>Bed making where care needs requires</li> <li>To prompt client to take prescribed medications.</li> </ul> <p><strong><em>General Duties</em></strong></p> <ul> <li>To work as part of the multidisciplinary team in order to deliver Home Support services</li> <li>To complete and maintain a daily work schedule as requested by the Home Support Resource Manager</li> <li>To be available for rostering for out of hours service by Home Support Resource Manager i.e. evening service, weekend duties.</li> <li>To adhere to Safeguarding Vulnerable Persons at Risk policy and procedure</li> <li>To use safely and efficiently, equipment provided in the work place</li> <li>To clean equipment provided to the client by CH CDLMS in the client’s home</li> <li>To carry out programmes as instructed by the relevant therapist and in line with the individual Standard Operating Procedure.</li> <li>To report promptly all faulty CHO1 equipment to identified staff member e.g. CRGN/PHN/Home Support Resource Manager.</li> <li>To adhere to safe procedures in the movement and handling of patients</li> <li>To adhere to guidelines on infection control</li> <li>To co-operate with the application of all policies and procedures of the HSE CHO Area 1 as applicable to the care of the client i.e. No Smoking Policy (place of work is in client’s home)</li> <li>In accordance with the Health &amp;
