Tel: 086 0439925 irene.cunningham@hse.ie</p> <p></p> <p><strong>Details of Service</strong></p> <p><strong>Donegal, Sligo, Leitrim, West Cavan</strong></p> <p>The Nutrition and Dietetic Service is made up of 29 WTE Senior Dietitians and 1 Dietitian Manager.</p> <ul> <li>The service provides clinical dietetic services to children and adults within primary care services as well as community hospitals throughout the counties of Donegal, Sligo, Leitrim and West Cavan.</li> <li>The service also provides nutrition support to patients on home enteral tube feeding.</li> <li>The service is involved in education of other health professionals and provides group education to service users including structured education as part of chronic disease management.</li> <li>In Sligo &amp; Leitrim we have Dietitians working in the Children’s Disability Network Team.</li> <li>In Donegal, Sligo and Leitrim we have Dietitians working within Mental Health Service in Eating Disorders</li> <li>In Donegal we have a Dietitian working in Integrated Care Team for Older Person.</li> <li>The service is involved in training of Student Dietitians.</li> </ul> <p><strong>Cavan Monaghan</strong></p> <p>The Nutrition &amp; Dietetic Service comprises of 9.0 WTE Senior Dietitians and 1 Dietitian Manager.</p> <ul> <li>The service provides clinical dietetic services to children and adults within primary care services throughout Cavan Monaghan.</li> <li>The service provides nutrition support to patients (enteral tube feeding and/or malnutrition).</li> <li>The service is involved in education of other health professionals and provides group education to service users including structured education as part of chronic disease management.</li> <li>We have dietitians working in Mental Health Services (CAMHs and AMHs)</li> <li>We have a Dietitian working in Ambulatory Hub (integrated Care Team for Older Person)</li> <li>The service is involved in training of Student Dietitians.</li> </ul> <p><strong>Reporting Relationship</strong></p> <ul> <li>Your professional reporting relationship for clinical governance and clinical supervision will be to the Dietitian Manager.</li> <li><strong>You will report operationally to relevant Operational Lead e.g. Community Health Network Manager, Chronic Disease Hub Operational Lead.</strong></li> </ul> <p><strong>Key Working Relationships</strong></p> <p>You will work in collaboration with other members of team as relevant to role to provide patient centred care in an integrated and coordinated way.</p> <p></p> <p><strong>Purpose of the Post</strong></p> <p>The Dietitian (Staff Grade) will function as an efficient and effective member of the dietetic team with the aim of ensuring optimum delivery of a clinical nutrition and dietetic service to clients, and ensuring appropriate follow up as necessary.</p> <p><strong>Pre Diabetes &amp; Weight Management Roles</strong></p> <p>The post holder will provide an evidence based clinical service for individuals with pre-diabetes and obesity in Primary Care. This service will support the implementation of the National Framework for the integrated prevention and management of chronic disease in Ireland 2020 – 2025 and associated models of care for the Management of Overweight and Obesity in Ireland and Type 2 Diabetes This will involve:</p> <ul> <li>Delivering evidence based HSE diabetes prevention programmes and weight management interventions within the network through a variety of platforms as appropriate.</li> <li>Supporting individuals to manage their weight to prevent progression of obesity, improve obesity related complications, and prevent additional complications.</li> <li>Providing a service in line with the model of care for individuals for whom a group programme is not appropriate using face to face or technology enabled solutions.</li> <li>Coordinating, communicating and reporting on HSE diabetes prevention and obesity management programme activities within the network.</li> <li>Working with the integrated care teams, SMS co-ordinators, health and wellbeing, mental health, disability, older person’s groups and social prescribing processes and the public to embed referral pathways in line with models of care and ensure integration across the services.</li> <li>Serving as a resource to assist individuals, other healthcare workers, or the community in relation to diabetes prevention and obesity initiatives.</li> <li>Working as part of a national team of educators – attending necessary updates, contributing to the development of national care plans, educator and client materials as required. Contribute to ongoing training and development of educators as required.</li> <li>You will be involved in student training.</li> </ul> <p><strong>Community Health Network Roles</strong></p> <ul> <li>The National Service Plan (2020) includes provision to commence a targeted reform programme in line with Slaintecare to Enhance Community Care (ECC) and redesign acute hospital pathways to support older people and those with chronic disease. This plan includes roll out of Community Healthcare Networks across each Community Healthcare Organisation (CHO).</li> <li>The post holder will provide an evidence based clinical service in association with other Dietitians and as part of a Multidisciplinary Team within a Community Health Network. This is a mapped network with a population of 50000 people You will be involved in a multidisciplinary team that</li> <ul> <li>supports people to live more independently in the community</li> <li>coordinates and integrates services to meet changing health needs of the population it services</li> <li>supports collaborative working to provide person-centred care</li> <li>ensures timely access to services hence reducing need for hospitalisation and for those who do require acute care post discharge support in the community</li> </ul> <li>You will deliver a dietetic service to adults/children in the community through individual consultations (in clinic setting or via telehealth), home visits and group education (in person or virtual).</li> <li>You may have a role in the provision of a dietetic service to residential units.</li> <li>Referral types ranging from chronic disease prevention and management to malnutrition, enteral feeding and specific clinical conditions.</li> <li>In addition, many dietitians support/lead on nutrition health promotion programmes in their area and develop resources and training for other health professionals on nutrition.</li> <li>In addition, there are regular team meetings, opportunities for CPD and research, ongoing support of student training.</li> </ul> <p><strong>Principal Duties and Responsibilities</strong></p> <p><em>The Dietitian Staff Grade will:</em></p> <p><strong>Professional / Clinical</strong></p> <ul> <li>Ensure that professional standards are maintained in accordance with the requirements as set out by CORU</li> </ul> <p><a href="https://coru.ie/files-recognition/standards-of-proficiency-for-dietitians.pdf" target="_self">https://coru.ie/files-recognition/standards-of-proficiency-for-dietitians.pdf</a></p> <ul> <li>Assist in the ongoing functioning and organisation of the dietetic service within scope of role and practice.</li> <li>Prioritise and manage a patient caseload according to the needs of the service.</li> <li>Deliver on an effective self-managed workload, maximising the use of time and resources.</li> <li>Identify nutritional problems through individual assessment of nutritional status and estimation of requirements.</li> <li>Instigate the Malnutrition Universal Screening Tool (M.U.S.T.) and relevant other adapted screening tools or resources for patient groups as relevant.</li> <li>Devise and implement appropriate nutritional care plans so that patients are assessed and advised appropriately.</li> <li>Liaise with catering staff (where relevant) in the development of menus and specialised therapeutic diets in accordance with national and local policies.</li> <li>Provide a dietetic service that is evidence based, encompassing specific objectives, strategies, audit and evaluation.</li> <li>Contribute to the development and implementation of standards and quality improvement initiatives.</li> <li>Participate effectively in multidisciplinary teams, team meetings and case conferences.</li> <li>Participate in research and in developing databases for relevant patient groups.</li> <li>Know the limits of own practice and when to seek advice / refer to another health professional.</li> </ul> <p><strong>Education &amp;
